To transfer from Kerbin's orbit, you will want your Apoapsis to intersect the Mun's orbit. If you want to reach the Mun on the first swing around, wait until Mun rises just above the horizon. A good rule of thumb is for each 100 km you are above Kerbin, wait another 2 seconds.

1- Get into orbit around Kerbin. It’s easiest if you have a nice, flat, equatorial orbit. 2 - Burn in the same direction you’re traveling to increase speed. Do this until your orbit shows an intersection with the orbit of the Mun and just a bit past it. 3. Speed up time and go round and round until you get an interaction with the Mun. 4.

The middle stage will carry your rocket from Kerbin Orbit to the Munar Orbit and might also help with your lunar landing. Put a TR-18A Stack Decoupler under the engine with a FL-T400 Fuel Tank under that. Use a LV-909 "Terrier" Liquid Fuel Engine for this stage. This stage should be at least able to carry your other two stages to Kerbin Orbit.

Create another maneuver node, this one on your orbit’s periapsis. Scroll on retrograde this time, opposite of prograde until your predicted orbit is a circle around the Mun. Execute this maneuver the same way as the previous one. Now that you are in Munar orbit, you will need to land on the Mun.
